Barcelona, Florence, Pisa, Gibraltar, Cannes, Cadis, Cartagena.

It depends on the port. Some ports are right in the town like Vigo, Cadiz, Monaco. Where the port is some distance from the town a complementary shuttle bus is provided. This would include Barcelona, Livorno, Lisbon. However, this would not include Rome from Civitavecchia, as the shuttle bus drops you at the port gate, However you can get the train from near there. Similarly for Pisa/Florence from Livorno using bus/train, although Florence in best done on a tour as it is some distance. In Gibraltar, the local taxi drivers operate a shuttle for £1 each way or you can walk in 15mins.
