There are many lifts between decks. Tours are mostly by transport although there are some walking tours particularly round old towns. The amount of walking is usually advised for each tour.
Yes, big modern ships have a dozen lifts or so covering the 12-14 (or so) decks. There are always excursions offered with minimal (and sometimes no) walking.
