On my last 12 night cruise on Queen Victoria we had 3 formal nights (Black & White Ball, Masquerade Ball and Victoriana Ball) two semi-formal nights and the rest were elegant casual. The dress codes for your particular cruise will be shown in your 'Pre-Voyage Documentation & Shore Excursions' booklet, which you should receive a few weeks before departure. Enjoy your cruise, Quuen Victoria is a lovely ship.
